McDonnel Douglas DC-10-10 accident report
Sunday 3 March 1974 Turkish Airlines DC-10-10 TC-JAV c/n 46704
ISP Flight TK981 from Paris (ORY) to London (LHR) with 11 crew and 335 passengers.
Cargo door failed when climbing out of Paris at FL 90, causing sudden decompression. The cabin floor collapsed as a result, hampering the flight control cables to the tail making the aircraft uncontrollable leading to the crash. The closing mechanism of the freight door had to be redesigned by FAA Airworthiness Directive.
First flight : 1972 Airframe hrs : 2955 cycles : 1537 engines : GE CF6-6D. All people on board died in the crash.

Friday 25 May 1979 American Airlines DC-10-10 N110AA c/n 46510
DSP Flight AA191 from Chicago (ORD) to Los Angeles (LAX) with 13 crew and 258 passengers. Take-off from RW 32R with 10° flap setting, TOW 379.000 lbs ( 171914 kg)
V1 139 kts, VR 145 kts, V2 153 kts. Just before rotation, the left (No.1) engine broke loose from the wing. TO was continued on 2 engines, and 9 seconds after lift off the aircraft had accelerated to 172 kts and reached a height of 140 ft. But while still climbing the speed fell back to 159 kts and then, at a height of 325 ft, the aircraft started to roll to the left and control was lost. The aircraft dived into an open field and trailer park 4600 ft NW behind the runway. First flight : 1972. Wrong maintenance caused the failure of the engine pylon structure on take-off. The outboard leading edge slats were retracted uncommanded leading to the loss of control at 159 kts.

Chicago O’Hare Runway 32R is not existing anymore. It was decommissioned during the airfield reconfiguration to reduce intersecting runways (O’Hare Modernization Plan OMP) which was completed in 2021.
Wednesday 31 October 1979 Western Airlines DC-10-10 N903WA c/n 46929
ISP Flight 2605 from Los Angeles (LAX) to Mexico City (MEX) with 11 crew and 77 passengers. First flight : 1973. Approach in poor weather for RW 23R, but touched down at RW 23L with the left landing gear besides the runway, a go around was attempted but the aircraft struck a truck located on the runway and then crashed. The aircraft was wrongly landed on a closed runway.
Airframe hrs : 24614 cycles : 7345 engines : CF6-6K. 72 people on board died in the crash and 1 on the ground. 16 injured.

Wednesday 19 July 1989 United Airlines DC-10-10 N1819U c/n 46618
DSP Flight UA 232 from Denver (DEN) to Chicago (ORD) with 11 crew and 285 passengers. While cruising at FL370, 1 hour and 7 minutes after take-off, the no.2 (tail engine) fan disk failed. Flying parts of the fan severed the hydraulic lines of all 3 systems and all hydraulic power was lost. An emergency descent was initiated to Sioux City airport (SUX). The aircraft landed on the 6600 ft long RW22 without flaps with a speed of 215 KIAS and a sink rate of 8.2 m/s. First the right wingtip touched the ground followed by the right main landing gear. The aircraft skidded to the right from the runway and turned inverted and then broke up leaving a long debris field till the main fuselage with wings came to rest just past RW35. First flight : 1973.
