ISP Flight CI140 from Taipei (TPE) to Nagoya (NGO), Japan with 15 crew and 256 passengers. ILS approach to Nagoya RW34 at night. The GO lever was inadvertently activated by the F/O which sets the aircraft in a go-around mode. The crew tried to correct the situation but the THS (Trimmable Horizontal Stabilizer) had got far out of trim with a high nose up AOA setting. The aircraft pitched up more and more steeply till it stalled. No recovery could be made from this stall and the aircraft crashed 300m beside the runway. First flight : 1990

Monday 16 February 1998 China Airlines A300B4-622R B-1814 c/n 578

ISP Flight CI676 from Depasar, Bali (DPS) to Taipei (TPE) with 14 crew and 182 passengers. Approach to Taipei apt RW 05L in light rain and fog ended to high, the flight crew initiated a go around but the aircraft pitched up at 42.7° without the crew notice, then the airplane stalled and impacted a residential area of Taipei, 200ft to the right of RW 05L. First flight : 1990. Airframe hrs : 20193 cycles : 8800 engines : PW4158. All 196 people onboard died in the crash and 7 people on the ground were also killed.

ISP Flight AA587 from New York (JFK) to Santo Domingo (SDQ) with 9 crew and 251 passengers. Take-off from RW 31L, 1 min 45 sec. after a Boeing 747-400. In the wake turbulence excessive rudder inputs were given which resulted in the vertical tail fin to brake off completely. The aircraft went out of control and crashed into Beach 131 street, Queens area. First flight : 1987. Airframe hrs : 37550 cycles : 14934 engines : CF6-80C2A5. All 260 people onboard died in the crash.
