BAC 1-11 srs 400 accident report

Friday 12 September 1969 Philippine Air Lines BAC 1-11-402AP PI-C1131 c/n 092

DSP Flight PR158 from Cebu (CEB) to Manila (MNL) with 5 crew and 42 passengers. The aircraft struck a hill in suburban Antipolo at 760 feet amsl during a VOR approach to Manila apt. RW24. YOM : 1966 Airframe hrs : 7208 cycles : 6445. 45 people died in the crash. A stewardess and one passenger were rescued.

ISP Flight from Tel Aviv (TLV) to Bucharest-Otopeni apt. (OTP) with 7 crew and 20 passengers. The aircraft flew into the ground during an NDB approach to Constanta in deteriorating weather (fog). The aircraft had diverted from Bucharest.YOM : 1968 19 people died in the crash and 8 injured. .

Charter flight AU009 from Buenos Aires (AEP) to San Carlos de Bariloche (BRC) with 5 crew and 74 passengers. Impacted a mountain while on approach to  BRC apt. RW24.

YOM : 1968 Airframe hrs : 21272 cycles : 22228 engines : Spey 511-14W. 46 people died in the crash and 33 injured.

Wednesday 26 June 1991 Okada Air BAC 1-11-402AP 5N-AOW c/n 094

DSP Flight from Lagos (LOS) to Kano (KAN) with 5 crew and 48 passengers. A weather front prevented the aircraft to continue to Kano. The aircraft diverted to Sokoto, 380km to the west of Kano, but at 21:00 hrs the airport was closed and lights were out. Fuel shortage eventually forced the crew to conduct a forced landing in open country. YOM : 1967 Airframe hrs : 35860 cycles : 39057 engines : Spey 511-14. 4 people died in the crash landing and 24 injured.
